The authors recount their adventures throughout April 1991. Cases described include a 15-year-old boy whose atopic dermatitis became must worse after eating rice, successful treatment of oral lichen planus in a 66-year-old woman, suppression of premenstrual acne flares in a woman, and more.

The Shelleys provide an interesting anecdote on biopsying: "We preach that taking a biopsy is like choosing a sample building to identify a city. A biopsy of a garage is no good. Look for a distinctive landscape feature. A 'biopsy' of the Empire State Building or the White House will tell you what city you are in." They also discuss the "epidermal trio."
